Persons who will have access to your information are you, administrators and agents of the State representing the Michigan Department of Health and Human Services (MDHHS), Bureau of EMS, Trauma and Preparedness (BETP); the Michigan State Police, Emergency Management and Homeland Security Division; federal, regional, and local partners involved in planning, investigating, or responding to a natural or public emergency; and the vendor contracted to maintain this site and services. Providing information to this registry is voluntary. By submitting information to the Registry, you agree and acknowledge that Michigan Department of Health and Human Services (MDHHS), Bureau of EMS, Trauma and Preparedness (BETP) staff and hospital administrators will review your information to verify your credentials as a volunteer. If you decide not to provide this information, we may not be able to contact you for volunteer work, training, or exercise opportunities.

I authorize the State to perform a criminal background check using the information I have provided.

The State may collect, use, and maintain my personal information to be used to support my participation as a volunteer only.

In the event of a declared emergency, would you work under the auspices of the Federal Government? If you check yes, in the event of a national emergency the information you provide will be made available to the Federal Government upon its request.

Do you have any medical limitations, which may affect your ability to volunteer in an event? This information is requested only for use in determining what reasonable accommodation, if any, may be necessary to facilitate your volunteers activities.

MI Volunteer Registry Privacy Policy

OVERVIEW

The State of Michigan and its agents, contractors, sub-contractors, and third party service providers (collectively, "the System Administrators") are committed to protecting the privacy of registrants and users of its products and services. The intention is to provide a safe and secure environment for MI Volunteer Registry (Registry) users.

The purpose of this Privacy Policy is to inform site users about the kinds of information the System Administrators may gather when visiting the site and how that information may be used. Please note that this policy applies only to the site and site content on www.MIVolunteerRegistry.org. It does not cover external Web Sites users may link to from the site.

ENHANCED SECURITY

This Registry site uses Secure Sockets Layer (SSL) technology to encrypt and protect data transmitted from its site. Alert messages and attachments that are distributed beyond the security of the site via unsecured email, phone or fax may be vulnerable to interception by unauthorized individuals.

INFORMATION ABOUT ALL SITE VISITORS

In general, the Registry site automatically gathers certain usage information such as the frequency of visitors to the site. Only such data would be used in aggregate for making improvements to the system.

The site uses a technology nicknamed "cookies" that is inherently a feature built into most internet browsing software. Cookies track how and when site pages are visited, and by how many people. Unidentifiable user data may be presented for statistical "ratings" information to site partners about how the site is used.

DISCLOSURE

Information about a specific individual that visits the site or their personal location information will not be used or disclosed. Points of contact like phone numbers, text pagers and email addresses will be alerted according to System Administrator designated roles and privileges. System Administrators may share such information in response to legal process (such as a legal violation, court order or subpoena) or in special cases where there is a risk of physical threat.

SITE PRIVACY POLICY CHANGES

Any changes to the privacy policy will appear in the Privacy Statement section of this site.

Administrator Turnover Policy

Registry System Administrators grant administrative access to designated representatives at the state, regional, local, and hospital level. In the event that a currently registered state, regional, local, or hospital administrator leaves their current position; it is their responsibility (prior to departure) to notify the Registry System Administrator at (517) 335-8150 or at help-MIVolunteerRegistry@michigan.gov.  The System Administrator will then reset the administrator’s account level as appropriate.
